ACCESS at Gary Comer Youth Center

Gary Comer Youth Center is pleased to host a health clinic operated by Access Community Health Network.

Access Community Health Network's services are designed to address the health of patients in preventive care, chronic disease management, and support services. To address patients' comprehensive health needs, physicians, nurse practitioners, are teamed with outreach staff, case managers, social workers, and substance abuse counselors to advance a continuum of care.
